Community Mental Health Practitioner
Mid and South Essex, Older Adult Admission Avoidance & Home Treatment Team Band 6 Salary Range £33,922 - £40,872 per annum (Including Fringe HCA)Main duties of the job
We are pleased to offer the new and exciting opportunity to join the new Mid and South Essex, Older Adult Admission Avoidance & Home Treatment Team. As part of the transformation agenda a new service is being formed to provide support to Older Adults, with Frailty and Functional Mental Health needs.
