Job summary
The Gables Specialist Mental Health Team is a multi-professional community team providing a range of mental health and social care interventions to people between the ages of 18-70 years with a diagnosis of a Serious Mental Illness (SMI).
The purpose of The Gables Specialist Mental Health Team is to provide help for adults experiencing the acute symptoms of a range of mental health difficulties, the main criteria for involvement with the service is that the person has a level of risk associated to their symptoms of SMI that cannot be managed within the primary care services (GP and Primary Care Network).

If you have been successful at the interview stage, but do not have enough experience for this role, you may be offered a position as a Band 2 Heath Care Assistant Apprentice. As part of the Band 2 HCA role you will be enrolled onto the Apprenticeship scheme, providing you with the opportunity to earn as you learn. This role enables you to gain valuable experience on the wards/community settings whilst working towards gaining your Care Certificate and naturally progressing into the guaranteed Band 3 HCA role.
Main duties of the job
- Some of your typical duties as a community support worker may include (but are not limited to):
- Supporting the qualified clinicians with developing and implementing care plans and co-working complex clients
- Support with the running physical health check clinics and promoting a healthy lifestyle clinics
- Encourage and support service users to engage with their local community
- Assist Senior Clinicians in the risk assessment and management of complex cases
- Identify any possible safeguarding concerns and escalate to your supervisors
- Support the promotion of carer and service user involvement within the service and participate in training associated with this
- Completing carer's assessments and facilitating carers groups
- Attend team meetings i.e. referral/zoning/business as appropriate
- Support service users to identify strengths, personal interests and achievements and work towards meaningful recovery goals.
- Produce accurate and timely documentation/records on all service users in line with relevant codes and Trust policy
ADDITIONAL DUTIES
In addition to the above duties you will also be expected to perform the below key activities in line with your job role;
Actively take part in service development.
- Complete mandatory training in line with Trust policy and procedures
- To participate in the staff appraisal process and to undertake for any staff you manage
- To keep yourself updated on all matters relating to Trust policy
- To provide management supervision where appropriate

As a Community Support Worker, you will work within the multidisciplinary team across the Braintree and Maldon districts to support people who are under the care of the team. You will look to address the physical health and wellbeing needs of individuals within a recovery based model.
You will be actively involved in engaging with people experiencing higtened symptoms of SMI (schizophrenia, bi-polar, personality disorders, depression, anxiety, OCD and others) and their families and supporting qualified members of the team to offer planned interventions recommended by NICE clinical guidelines. This will include but is not limited to: monitoring mental health, social inclusion, practical planning with life skills, managing risk, offering family interventions and carers assessment and input.
In addition it is anticipated that the worker would support with Physical Health Checks if required, working alongside the teams Associate Practitioners. This will involve the assessment and recording of physical health and lifestyle, action planning, as well as development and implementation of activities and lifestyle action plans either 1:1 or in a group setting. You will have the support and supervision from Band 6 colleagues and Clinical Lead.
You will be able to draw on the interventions and expertise provided by your MDT colleagues such as: Support Workers, Community Psychiatric Nurses, Social Workers, Psychologists, Employment Specialists, Peer Support Worker and Medical Colleagues.
