Job summary
This exciting senior management post has been developed to have oversight of the West Essex Care Coordination Centre (CCC). The post holder will have overall responsibility for the day to day operational management of the CCC supporting the delivery of a high quality responsive service to ensure that patients receive the right care, in the right place at the right time and that care is coordinated in partnership with other health and care providers across the system.
Main duties of the job
To provide vision and professional leadership to the integrated CCC team facilitating professional development and promoting workforce wellbeing in line with Trust policies and procedures. To deliver integrated care in partnership with colleagues across acute Trusts, social care and Mental Health to improve user choice and experience. To be responsible for enabling the efficient and effective provision of high quality community based integrated care to appropriately referred patients. To assist the senior operations Manager in the strategic planning, development and delivery of services across the service area to include risk stratification and population health management.

- To ensure that there is a robust communication strategy across the CCC that links with the West Essex Care Unit and Trust
- To represent the Trust on groups with direction from the Senior Operations Manager with a range of internal and external stakeholders and agencies
- To utilise a range of communication skills including negotiation, listening skills and preparing and delivery of formal presentations to a wide range of stakeholders
- To communicate a range of complex, sensitive and contentious information in a range of formats including one-to-one meetings, formal and informal meetings, large groups, written formats (reports, letters), and electronic communication (e.g. email, databases and reports)
- To liaise with all members of the integrated neighbourhood teams, working collaboratively to ensure support to meet the persons and or carer needs are coordinated, and timely to ensure a positive experience. met in a timely and coordinated service, resulting in the timely
MANAGERIAL
- To provide operational management support to the clinical integrated CCC team and work in partnership with the Administration Manager for the CCC.
- To provide leadership to the team including supporting the team through change, motivating the team, role modelling team working and collaboration with a solution focussed approach, encouraging ideas from all team members both professional and administrative; supporting learning and development in order to establish a culture of continuous improvement across the CCC-
- To provide leadership support to the CCC team and actively participate in projects and programmes of work that are agreed as part of the CCC operating model
- To supervise and carry out appraisals with senior staff within the CCC integrated team to identify their learning and development needs and link their objectives to the needs of those of the service, and the West Essex Care Unit strategic plan To ensure that all staff within the team have appraisals on an annual basis
- To be responsible for ensuring staffs attendance at mandatory training sessions as specified by trust policy,
- To ensure that staff are competent to carry out their role and meet specified professional standards. Identify when additional training is required and resources to support it
