You will be expected to work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within a multi-disciplinary team system with the responsibility for delivering bespoke care package interventions to those service users who meet the criteria of community mental health treatment.
Main duties of the job
To be responsible as the senior community mental health nurse for the assessment and coordination of care needs, planning and implementation of evidence based care to a defined group of service users and their carers within a defined catchment area/locality. To provide professional advice and support to other agencies and members of the multidisciplinary team, to act as a role model and resource for junior/less experienced staff.
