An exciting opportunity has arisen for you to join the NE Dementia Service Community Team. We are seeking an enthusiastic individual who has a caring and compassionate approach to their work. The post holder will be expected to provide support to the Registered Nurses within the team, visiting people with a diagnosis of dementia or experiencing memory difficulties within their home setting, Monitoring medication side effects, arranging medication review where required, covering duty and triaging referrals. Applications welcome from Associate Practitioners and/or Associate nurses.

Main duties of the job

  • Supporting the qualified clinicians with implementing care plans and completing risk assessments
  • Monitoring medication following commencement of Dementia Medication for patients and arranging medication review where required.
  • Liaising with Primary care for better management of Patient physical health
  • Covering single point of Access (duty) by triaging Referrals into the service
  • Running physical health check clinics and promoting a healthy lifestyle
  • Engage with service users to develop activity programmes or 1:1 interventions that meet their needs
  • Encourage and support service users to engage with their local community
  • Identify any potential risk and escalate any concerns to your supervisors
  • Identify any possible safeguarding concerns and escalate to your supervisors
  • Support the promotion of carer and service user involvement within the service
