Job summary
The community team of West Essex is part of the wider Hertfordshire and West Essex Integrated Care System supporting Out of Hospital Care to support West Essex Residents. The post holder will be working as part of the Integrated neighbourhood teams supporting Uttlesford North and south with a focus on prevention of admissions, supporting safe discharge as well as provision for rehabilitation for the West Essex population.
Main duties of the job
Clinical responsibilities
- To deliver high quality, skilled clinical care to patients at home and provide rehabilitation and long term care/conditions management in a variety of community settings.
- Autonomously manage a caseload of patients with complex health & social care needs, using evidence based, client centred principles to assess, plan, implement & evaluate interventions
- To utilise specialist skills and knowledge to holistically assess needs and provide programmes of rehabilitation based on evidence based practice.
- To select appropriate treatment techniques from a wide range of specialist skills available. The selection of techniques will be based on both theoretical and practical knowledge and evaluated past experience.
- To develop collaborative treatment programmes with patients, carers and colleagues, demonstrating client centred reasoning and specialist problem solving techniques.
- Plan and prioritise own workload and delegate tasks where appropriate to ensure effective use of time.
- To work within and across professional and organisational boundaries, liaising with GP's other professionals and statutory and voluntary agencies to deliver individualised and expert clinical care to a high standard to clients.
- To take responsibility for managing the caseload and the team, in the absence of the band.

- Using verbal, non-verbal and written communication tools ensure that patients and carers have an understanding of their condition and the role and aims of Occupational Therapy in relation to this. The Occupational Therapist must effectively amend their communication techniques according to individual needs of patients (for example the patient may have a learning disability, sensory problems, neurological difficulties or may not use English as their first language).
- To be confident in their own diagnosis and be able to communicate that effectively to others (including medical doctors, other Allied Health Professionals etc).
- To share information in a timely manner with more senior staff if there are any concerns about a patients safety, condition or prognosis.
- To urgently report and take, appropriate action if there are any concerns about staff and/or patient safety to the more senior therapy staff.
- To represent the community service in meetings with other professionals with an aim of accurately contributing and reporting back relevant information to their line manager.
- Maintain confidentiality at all times, consistent with Trust policy.
- Maintain accurate clinical records in accordance with professional and trust standards.

- The Occupational Therapist must take a holistic view of the patients and their wellbeing considering all the factors and not purely focus on a specific medical condition, sign or symptom.
- To be professionally and legally accountable for all aspects of own work including the management of patients in your care.
- To take a lead for specific projects within your area of work to enhance patient care and service delivery.
- To train, supervise and performance manage junior staff, technical instructors, assistants and students (some on a daily basis) with assistance from more senior members of the physiotherapy team when needed. This will include the use of formal appraisal documentation.
