Job summary
We have an exciting opportunity for a Registered Nurse RMN/ Social worker/OT to undertake the role of Discharge Coordinator within the MID Essex location.
The inpatient wards provide assessment and treatment of service users experiencing a range of mental health problems. The team looks at planning for effective and timely discharge by working within the multi-disciplinary team and takes the lead for discharge planning of complex cases. The Discharge Coordinator role is necessary to improve the flow of service users through their inpatient journey by supporting the service user with care packages, accommodation choices, funding applications and decisions thus avoiding unnecessary delays. The role requires the discharge coordinator to reliably communicate the needs of the service user and ensure action between inpatient services, community mental health services and other key agencies is efficient in order achieve timely, appropriate and effective discharges from hospital.
The role will involve preparing and presenting cases to health and social care panels ensuring all necessary assessments have been completed. The role includes assisting inpatient staff with the necessary assessment paperwork and processes and you will work closely with health and local authorities in identifying suitable placements and packages of care where necessary.
Main duties of the job
- Will act as an independent practitioner within hospital settings, while liaising with other professionals as necessary.
- Will work with the MDT identifying barriers to discharge, and how these can be resolved, ensuring the safe discharge of the individual presenting to the acute hospital setting
- Will be responsible for supporting the assessment, planning, implementation, and evaluation of nursing care within agreed parameters, to ensure meaningful admission and safe, appropriate discharge.
- Have a working knowledge of local, national policies and legislation, which govern current service provision and audit.
- Will be responsible for ensuring effective communication pathways exist.
- Will be responsible for ensuring the promotion of equality diversity and rights in accordance with good practice and legislation.
- Will be responsible for identifying and contributing to the training needs necessary in providing quality evidence based care as appropriate.
To provide professional advice and support to other agencies and members of the multidisciplinary team, and act up for the Team Manager in their absence as appropriate. To act as a role model and supervise less experienced staff.
