The service has been developed in partnership to provide a full multidisciplinary crisis response to individuals in their own home or care setting, to support the NHS longterm ambition of preventing all unnecessary hospital admissions and ensuring delivery of care by the right person, in the right place at the right time.

Our EPUT NHS staff bank is an entity managed by the trust that hires clinical and non-clinical healthcare professionals to take on shifts at our trust hospitals and community settings. Here at EPUT we maintain our own bank of specialist staff to ensure that we are able offer safe and effective care at all times.

All our permanent staff are automatically enrolled onto the staff bank however this does not mean you have to work any additional shifts, but the option is there for you if you wish.

The post holder will work within the Urgent Community Response Team (UCR) as part of thewider Integrated Care Team to assist in the prevention of inappropriate hospital admissionsand the facilitation of hospital discharge in line with the overall objectives of the service.Responsibilities will include, carrying out initial assessments and planning care for patientson the Urgent Community Response Service who are in crisis in need of rehabilitation andreablement following discharge from hospital or to prevent hospital admission. Usingevidence-based/patient centred principles to assess, plan, implement and evaluateinterventions, as well as responsibility for managing and transferring an individual patient'songoing care to other professions and agencies as appropriate.The post holder will work in partnership with patients, carers and other health and social careagencies to promote independence and develop individual plans of care relevant to patientneed.You will be expected to work multi professionally, support other team members roles,contribute to the future development of the service providing professional advice andappropriate training to others in the multi-disciplinary service.The post holder will work collaboratively with all partners within the Primary Care Networkand as part of the PCN Aligned Core Team to meet the needs of the local population usingpublic health information to support service development and a more seamless connectionbetween all Community Partners.he post holder will be expected to work within a flexibleworking pattern over a 7 day week.
