Job summary
To be responsible for a clinical caseload of complex and non-complex patients including those with palliative care needs, orthopaedic conditions, neurological conditions, COPD and other pulmonary conditions. To be able to predict disease progression and consider this in clinical reasoning some patients may also have dual diagnosis that impacts on their physical conditions, such as dementia.
Main duties of the job
To work with adults in the community, providing OT assessments, intervention /treatment and evaluated input. The focus of the service is to prevent hospital admission and support successful discharge, through assessment of the person and their social and physical environment. Then to provide therapeutic input that maintains functional ability and promotes rehabilitation, using compensatory techniques, adapting the environment and providing recommendation of support needs
