Job summary

The Harlow North Primary Care Network (PCN) is at the forefront of community mental health transformation. As an innovator in integrated holistic patient centered care the PCN champions equality of access to and delivery of mental health services for it's population. The PCN is already supported by an Integrated mental health practitioner, mental health coach and community mental health pharmacists. The Mental Health Practitioner Additional Role within the PCN will provide the post holder with a unique opportunity to develop and build integrated mental health services tailored to the needs of the Harlow North community.

Working as part of both Essex Partnership University Trust and the Harlow North PCN the post holder will be part of highly motivated and innovative organisation committed to improvement effectiveness and safety.

Working at the forefront of primary care and community mental health transformation the post holder will be in a unique position to help shape the future of Mental Health Services within Harlow North.

The post holder will be supported to develop through a commitment to professional development and regular supervision and appraisal.

Main duties of the job

You will be responsible for the clinical management of patients with a mental health presentation within the Harlow North Primary Care Network (PCN).

Be an active and integral member of the PCN MDT contributing expert knowledge to the formulation and delivery of care and treatment plans.

You will contribute to the development of new ways of working as a key member of the primary care network. This post will involve working with other agencies to support the development of resilient communities and the management of good mental health in line with the West Essex One Health and Care partnership objectives and the domains and the NHSE Long term plan.

Working collaboratively with all members of the PCN team to deliver holistic and recovery focused intervention to the residents of Harlow North PCN.

Ensure that services are accessible to all ensuring modes of delivery are flexed to enable access by all communities focusing on those who have historically been difficult to engage. Participate in and provide skills and knowledge training for the wider PCN roles in awareness, care and support of those with mental health needs

Work in collaboration with the voluntary, third sector and local district councils to support the development of mental health accessible and informed services across all areas of the Harlow North community.

The post holder will facilitate the management of flow and capacity at a primary care level and the interface with secondary care mental health and the voluntary sector.

The post holder will work with the wider health and social care economy including non-statutory and statutory organisations in the creation of integrated care plans to support people in their communities.

Developing and supporting The lived experience agenda with Patient and Carer engagement and involvement in service transformation and delivery is an integral requirement of this post.

This post requires the delivery of direct patient assessment care and management functions in collaboration with the wider PCN team.

Core Clinical functions include

  • Contribute fully to the integrated care agenda within the primary care networks.
  • Engage in a systematic case discussion process with all PCN/community/Social Care staff.
  • Support staff with complex cases, facilitating MDT meetings, and other forums as required.
  • Produce accurate and timely documentation, conduct and manage the maintenance of accurate and timely documentation / records on all service users in line with your Professional Code of Conduct and Trust policy and the contractual requirements
  • Conduct and supervise assessment and observation to ensure appropriate and timely feedback to the team
  • Provide professional clinical leadership at multi-disciplinary meetings i.e. referral / review / business, professional or safeguarding meetings
  • Report and document incidents as required and act upon findings as appropriate
  • Provide education and training packages to support and develop patients understanding of mental illness.
  • Provide practical support, guidance and education for services users, carers and other professionals as required
  • Lead and actively take part in comprehensive assessment, care planning, treatment and review of care
  • Lead and participate in formulation, delivery and review of individualised evidence based care / care packages according to NICE guidance including non-medication strategies
  • Lead in the continuing development of a high quality service clinically underpinned by evidence based practice, care pathways, NICE and DH Guidance
  • Identify, assess and manage clinical and non-clinical risks / hazards in your area through the completion of the relevant risk assessments and the implementation of risk treatment plans for inclusion in the local and Trusts risk registers
  • Contribute to the locality/PCN emergency plan
  • Understand the eligibility for Care Act Assessments and ensure appropriate assessments are undertaken.
