Job summary

The role of FIRST is to prevent acute psychiatric hospital admissions for older adults aged 70 years and over who are presenting in acute crisis which may be complex in nature where mental health and related physical health conditions can be managed within the community between the operational hours of 8am-8pm. FIRST provides responsive assessments and proactive home treatment. This includes joint working with the MDT, Older adult CMHT's, Home treatment teams, GP's, inpatient wards and all other partner agencies including the voluntary sector.

FIRST will gate keep all Mental Health inpatient admissions and facilitate early discharge should a hospital stay be no longer required and treatment can be continued safely in their place of residence.

Main duties of the job

You will be working as a senior member of the multidisciplinary clinical team, working in one specified pathway or across pathways in accordance with area / locality structures, assessing and treating your own complex specialist caseload of service users in accordance with Trust clinical pathways.

You will lead with other senior colleagues in the continuing development of a high quality service that is underpinned by evidence based practice and care pathways / packages and NICE guidance and DH guidance; leading on service development and policy in conjunction with the other Clinical leads and Clinical Managers.

You will also have responsibility for the day-to-day supervision and management of lower banded staff within your professional group and team locality. As required by your manager this may include the management of these staff in line with agreed Trust HR processes.

Job description

  • Responsible for providing and overseeing specialist advice on equipment, telecare etc. to other disciplines
  • Responsible for and lead on induction of staff and fieldwork education of students
  • Act as Care Coordinator for highly complex cases
