Job summary

We have an exciting and challenging opportunity for an experienced and motivated individual with a demonstrable interest in working with patients who are living with a mental illness in the community.

We are seeking a dynamic and enthusiastic individual who is committed to working in partnership with patients/carers and a range of other care staff and agencies to provide truly person centred services.

You must have excellent communication skills, commitment to working flexibly to meet patient's needs, be able to demonstrate that you can work effectively in a multi disciplinary team setting and demonstrate a commitment to further development with the support of the team. You must have an awareness of current health and social care issues that affect community care for Adult with mental health needs.

You will have commitment to your own development including experience of a varied caseload, health promotion and clinical supervision. Your active participation in providing a variety of learning opportunities for students and participating in training initiatives in general will be a part of the role.

The post also offers the opportunity to actively influence and participate in the development of services to Adult with mental health needs living in the South West area.

Main duties of the job

The post holder involves care coordination within Recovery & Wellbeing Team for those patients that present with severe and enduring mental that might at complex for primary care services to manage. The post holder will be responsible for providing high quality, effective care and management of the these cases and at times helping with depot clinic, clozapine clinic and physical health checks if required. They will be a member of a multi-disciplinary team and work in close co-operation with hospital based and primary health care based professionals. They will be responsible for the maintenance of standards of care within the Recovery & Wellbeing Team and actively participate in the training and education of students and staff seconded to the service. In addition, they will also be responsible for ensuring that an integrated assessment and treatment service is provided to the clients and ensure that a good inter-disciplinary working environment is maintained.
