A rare opportunity has arisen for a Team lead in the Dementia Intensive Support Team in North East Essex. This team provides a range of supportive interventions in the persons home to prevent and manage crisis. DIST has been in operation for around a year and we are looking for someone who can continue the good work, innovate new ideas, drive service improvement and work collaboratively with a number of individuals and organisations to reduce the number of admissions for this group.
Applicants will be expected to have professional registration and relevant experience at B6.
Main duties of the job
The Dementia Intensive Support Team provides urgent support interventions in the community to support people with dementia and their carers in crisis and to prevent crisis. Minimising the need for admissions to both general hospital and mental health inpatient settings.
You will have continuing responsibility for proactive assessment, monitoring and management of patients and ensuring care is current, evidence based and in line with practice standards. You will provide clinical leadership to the team in evaluation and monitoring of the service, provide specialist assessment as per the service users, and carer's requirement
You will be expected to work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services; providing a 7 day a week, 24 hours a day service for 365 days a year working shifts where appropriate.
In delivering your duties you are expected to display behaviors that are aligned with the Trust values and as required in accordance with the NHS Constitution at all times.
