Job summary

We are looking for enthusiastic, innovative and motivated individuals to join the West Home First Team as Band 6 Senior Community Nurses. The posts are based at the Derwent Centre in Harlow and covers a geographic area to include Harlow, Epping, Loughton and part of Uttlesford.

The Home First Team provides intensive home treatment to mental health clients in the community and also provides a gatekeeping service to adult mental health admissions. The team facilitates comprehensive multi-disciplinary assessments, identifies individualised care pathway needs and when identified as required provides onward referral to acute and specialist services, working closely with all services involved. This is a varied role which provides a high quality community and urgent service to the local population.

You will provide high quality, evidence based holistic nursing care to service users in line with your NMC Code of Professional Conduct, Trust policies and NICE guidelines.

This is an exciting opportunity for those interested in supporting acutely ill clients in their homes. You will be working collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services providing a 7 day a week service for 365 days a year working shifts where appropriate and operationally required.

Main duties of the job

ROLE SUMMARYYou will work as a member of the nursing team in providing high quality, evidencebased, holistic assessments and nursing care to patients in line with your NMC professional code of conduct and Trust policies.You will contact patients following referral, conduct urgent and routine assessments, develop and agree collaborative care plans, and deliver a range of brief interventions based upon individual service user needs.

Assessments and interventions can take place in patient's homes, team bases, hospital and other community settings.

As required by your manager, you will have responsibility for the day to dayallocation of the team, supervision and management of lower banded staff within your professional group and team locality. This will include the management of these staff in line with agreed Trust HR processes.

You will be expected to work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services; providing a service 8am - 9pm, 7 days a week, for 365 days a year, working shifts where appropriate and operationally required.
