Job summary

The post holder will be responsible to the Service Manager of Older Adult Community Mental Health, Dementia & Frailty (Mid & South Essex). The post holder will be responsible for the management of the FIRST (Functional Intensive Support and Response Team) admission avoidance, crisis and home treatment pathways, providing Leadership, supporting staff and working collaboratively with Consultants Psychiatrist in Mid & South Essex, other grades of Doctors, Allied Health Professionals and Social work Practitioners to provide and deliver a high quality and safe care.

FIRST has commenced operation in South West Essex (Basildon, Brentwood and Thurrock) and commences in Mid Essex in September 2022. The final stage is for the service to cover South East Essex.

FIRST will align with DIST (Dementia Intensive Support Team) in South East Essex.

The post holder will demonstrate critical thinking in the clinical decision making process, including joint working with the Multi-Disciplinary Team to meet the needs of the patients, and carers to ensure that they are working to EPUT Clinical Policies and Procedures, Nursing and Midwifery Council Code of Practice and Standards. In delivering your duties you are expected to display behaviours that are aligned with the Trust values and as requested in accordance with the NHS Constitution at all times.

Main duties of the job

  • To work collaboratively with Inpatient and Community (Mental and Physical Health) services in order to aid the Patient's recovery and Well-being plans in the community.
  • To ensure safe Clinical Practice takes place and maintained to Care Quality Commission standards at all times and in line with the NHS Constitution.
  • To work in conjunction with the Consultant Psychiatrists, to ensure that patients are supported in the most appropriate clinical practice environment from a Multi-disciplinary perspective
  • To create appropriate pathways within sphere of responsibility and work closely with the Assessment and Treatment units to provide a seamless service when an admission is warranted.
  • To manage and deliver effective and safe services which support the key elements of Clinical/Practice governance. The post holder will focus on leading the teams to introduce ways of continuously improving the quality of their services and safeguarding high standards of care by creating an environment in which excellence will flourish.
  • You will be expected to work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services; providing a 7-day a week, 24-hour a day service for 365 days a year.

Operational Management

  • The post holder will ensure that systems of care are in keeping with best practice and evidenced based.
  • Enable staff to assess, plan, implement and evaluate care with their patients, integrating Clinical/Practice interventions into a care package devised with the Multi-Disciplinary Team.
  • Use expert knowledge and experience when supervising staff and making decisions. Demonstrate consistent and clear decision making process underpinned by relevant theory.
  • Advise colleagues and staff (internally and externally) on patient issues utilising expertise and skill to promote holistic and therapeutic interventions in the management of risk.

  • To ensure that there are safe and effective shift co-ordination arrangements in place to ensure that resources are competently deployed and the demands of the services are met.
  • Carry out complex investigations in accordance with policies and procedures.

  • To manage human resource related issues such as sickness absence and conduct and capabilities of area of responsibility in accordance with EPUT policies and procedures.

Strategic

  • To work with EPUT and other relevant partners to map the mental health needs and identify/agree the priorities for the development Community Mental Health Service.
  • To work with other agencies to develop an integrated approach to the planning and delivery of services.
  • To assist in developing the 24/7 Crisis Care Pathway.
  • To support the Service Manager in the strategic development of Services.

Performance Management

  • To ensure that required referral response times are adhered to and targeted activity levels are delivered.
  • To be responsible for the delivery of corporate and service wide targets and objectives within the area of responsibility.
  • To ensure that team leads are effectively monitoring information on individual and team activity, ensuring daily diary sheets are completed for all staff and returned on time.
  • To ensure proactive engagement with inpatient services to enable supported discharge and reduce length of inpatient stay.

Human Resources

  • To manage staff and team performance in line with EPUT policies and procedures.
  • To support recruitment and selection processes in their areas of responsibility (in conjunction with professional heads as appropriate).
  • To ensure the optimum deployment of staff in their areas to meet operational requirements (in conjunction with professional heads as appropriate).
