The service has been developed in partnership to provide a full multidisciplinary crisis response to individuals in their own home or care setting, to support the NHS longterm ambition of preventing all unnecessary hospital admissions and ensuring delivery of care by the right person, in the right place at the right time.

Our EPUT NHS staff bank is an entity managed by the trust that hires clinical and non-clinical healthcare professionals to take on shifts at our trust hospitals and community settings. Here at EPUT we maintain our own bank of specialist staff to ensure that we are able offer safe and effective care at all times.
All our permanent staff are automatically enrolled onto the staff bank however this does not mean you have to work any additional shifts, but the option is there for you if you wish.

You will assess, plan, implement andevaluate packages of treatment and care based upon individual service user needs, underthe guidance of the Advanced Nurse Practitioner as per NICE guidance.The Urgent Care Response Team is a new service to provide a 2 hr response to patientswho are in a mental or physical health crisis, who without the 2 hr response are at risk ofbeing admitted to an inpatient ward inappropriately. The MDT will comprise of Mentalhealth nurses, Community nurses, Social care staff, Occupational Therapists andPhysiotherapists who will work collaboratively to ensure an out of hospital treatmentpathway is considered as an alternative to admission if clinically appropriate.As required by your manager, you will have responsibility for the day to day supervision andmanagement of lower banded staff within your professional group and team locality.
