The Isle of Dogs Neighborhood Community Mental Health Team is part of the South East Locality catchment are of the London Borough of Tower Hamlets whichcovers Primary Care Network 7 and 8. We are an established Mental Health team with motivated, proactive and supportive team members. We care for service users with complex needs, including severe and enduring mental health needs requiring us to provide skilled assessment and interventions through the integrated Health and Social Care model of care.
This is an exciting opportunity for the successful applicant to develop their understanding and delivery of Health and Social Care. The successful applicant will work alongside a multi-Disciplinary team and will co-locate with partner organisations to ensure that provision of health and social care is delivered in a coherent and integrated pathway.
The Team promotes opportunities for professional development through regular supervision, access to training, including managerial and leadership and quality improvement (QI) projects.
Main duties of the job
The post holder will be responsible for:
- Work closely with PCNs/GPs, Social Services, Housing and other statutory and voluntary agencies to navigate care pathways and ensure that service users benefit from person-centered care and support plan.
- Actively engage service users and their carers in all stages of care provision, ensuring that their views and what matters to them are acknowledged and taken into account.
- Be part of the Duty Rota for the Team, in line with responding to queries as they arise.
- Undertake preparation and producing of statutory and other reports, and conducting other delegated responsibilities on behalf of the Team.
- Have the responsible for the administration of depot medication and other clinical interventions.
- Participate in quality improvement initiatives and projects, audits and surveys relevant to the Service.
- Be part of Continues improvement to ensure quality of service delivery is maintained.
- To proactively engage in supervision and appraisal and provide supervision to junior colleagues.
