Specialist Nurse – Children’s Continuing Care Team

Job summary

Due to a team member being seconded to a course, we are looking for an experienced innovative clinicalnurse with extensive expertise in paediatric nursing, to work in our Continuing Care Team alongside colleagues. Our service sits within a wider multi-disciplinary service including Nursing (Specialist School Nursing, Asthma, Epilepsy, CCNT, Enuresis, Diana palliative care team) and community paediatric dieticians. You will be delivering an effective and person-centered service for children, young people and their families with the highest level of medical needs in the borough, including those that are technology dependent, highly complex and/or requiring palliative care, assessing and supporting with packages of care in the community.

You will have excellent organisational and interpersonal skills, experience of leadership and be able to work within a senior management team to support the wider service.You should be highly motivated, caring and passionate about improving the healthcare and wellbeing of children and young people in Newham. Main duties of the job

The post holder will play a key role in managing the Children's Continuing Care caseload within the Community Children's Nursing Service in Newham, working within the remit of the current Children and Young People's Continuing Care National Framework. This includes delivering an effective and person-centred service for children, young people and their families with the highest level of medical needs in the borough, including those that are technology dependent, highly complex and/or requiring palliative care.

The post holder will play a key role in leading and developing the Continuing Care Team, working in conjunction with the Children Community Nursing Team (CCNT), Diana Palliative Care Team and external stakeholders, to implement effective service pathways, ensuring the delivery of key quality outcomes.

An essential element of the role will include seamless working with other professionals and partner agencies when assessing eligibility for Continuing Care and supporting multidisciplinary teams in the assessment process. The role will ensure that the provision of care achieves individual outcomes and ensures the delivery of efficient quality packages of care that is aligned to the Personalisation and SEND agenda and supports the transition of young people into the appropriate adult services.
