Job responsibilities

There are two posts available based in Shefford Health Centre to deliver care to patients in Mid Beds and Steppingley Hub to deliver care to patients living in West Mid Beds.

The successful candidate will be expected to be flexible and assist colleagues to cover service needs, which will include working across the wider integrated team. Access to a car in order to carry out service duties is essential.

A full induction programme will be provided.

The Community Nursing Service operates between 08.00 to 21.00 hours and the working pattern will include a variety of rotational 7.5 hour shifts which will include weekend, evening and Bank Holiday working.

• oTo carry out any other clinical duties under instruction from senior staff within the Integrated Community Services covering other staff within the service to meet patient needs.
• oEnsures all patients on caseload have an up-to-date care plan carried out by a qualified health care professional.
• oWorks with the extended primary care team to work through the patient-focused care plan.
• Understanding of the importance of following procedures and treatment plans
• Good verbal and written communication skills
• Ability to assess, plan, implement and monitor care
