Job summary

Are you a Band 5 Nurse who would like to join our dynamic team? We are seeking an ambitious, motivated, compassionate and experienced Band 5 nurse to support the changing face of community health care.

Main duties of the job

NOT FOR ADVERTISEMENT

Through face to face patient visits you will undertake a full holistic assessment, plan care with the patient at the centre and undertake physical examination of the patient with undifferentiated conditions.

You will be working closely with other members of the MDT and partners to formulate a management plan that will support the patient to remain in the community

Experience in the field of adult community services is desirable but a willingness to learn and contribute is more important. A flexible approach is required to accommodate the changing needs of the service along with the drive to influence service development.

You must have a driving license and have access to a vehicle in order to carry out your duties. You will be required to work a shift pattern covering 7 days a week from 08:00-20:00.
