Community Mental Health Nurse

Join our dynamic team as a permanent Community Nursing and Care Coordinator in the vibrant Bethnal Green and Globe Town Neighborhood Mental Health Team! This role offers a fantastic opportunity to make a real impact in the lives of individuals facing mental health challenges.

In this position, you will conduct thorough assessments within Health (SAP/CPA) and Social Care (Care Management) frameworks, allowing you to develop, implement, and continually refine tailored care plans. Your expertise will be invaluable in supporting and advising family members and caregivers, ensuring they have the guidance they need.

We believe in championing the voices and needs of our service users, and we're eager for you to promote their well-being and provide training and education wherever possible. Collaborating closely with crisis pathway services, you'll play a crucial role in delivering a safe and responsive urgent community assessment and treatment service.

You'll conduct comprehensive risk assessments with efficiency and effectiveness, working collaboratively to formulate and agree on crisis safety care plans. Your supportive approach will make a difference to relatives and caregivers during critical times. Main duties of the job

To assess, plan, set goals with the service user, carers and families; implement and evaluate care using psycho-social interventions and skills to meet individual needs.

To actively support the person in crisis in self management of their symptoms through the formation of their care plans.

To act as a care coordinator for service users.

To be responsible for accurate record keeping in line with Trust and NMC policy and procedures.

The post holder will contribute to establishing and maintaining liaison with the members of the primary/secondary healthcare team, voluntary agencies in the professional disciplines necessary to provide a comprehensive service to clients and their families

Complete assessment and discharge documents in a timely fashion

The post holder will have working relationships within a multidisciplinary team and the wider crisis pathway.

  • RMN Educated to degree level
  • Mentorship Training or equivalent
  • Recognised post graduate qualifications in care of the adult with mental health problems
  • Evidence of regular updating and ongoing development of role and skills

Knowledge and Skills

  • Thorough understanding and working knowledge of mental disorder and the treatment options. . Ability to assess manage complex and high risk situations effectively and to the benefit of service users and carers
  • Good report writing skills and ability to compile complex case recording ad report presentation, including legal documents i.e. social circumstances reports, court reports etc
  • Ability to manage and co-ordinate professionals, teams and services and mobilise provision where appropriate. . Assessment skills and the ability to compile care plans in partnership with service users, carers and others
  • Assertiveness and engagement with other professionals, able to make reasoned arguments and represent own professional views and perspectives
