Job summary
The Adult Speech and Language Therapy Service, based at East Ham Care Centre, Newham, provides assessment, treatment and management to a wide range of adult clients with acquired communication and / or swallowing difficulties in a community setting. The caseload is varied, and includes adults over 18 years of age, with acquired speech, language and swallowing difficulties secondary to, for example; dementia, other mental health conditions, respiratory conditions, cancer, palliative and end of life care.
The post holder will:
Use and continue to develop specialist skills and knowledge in the management of adults with acquired communication and swallowing disorders in a community setting.
Independently manage a large, complex and diverse clinical caseload, working closely with community colleagues across East London NHS Foundation Trust, and acute colleagues across Barts Health NHS Trust.
Act as link speech and language therapist to SLT and MDT colleagues within the local acute Trust, other ELFT services and external agencies, such as the multidisciplinary rapid response service, which focuses on preventing avoidable admissions to hospital, and the extended primary care teams, who aim to keep people at home where possible to improve the quality of people's lives.
Provide a service that takes account of the needs of clients from a range of ethnic and linguistic backgrounds.
Main duties of the job
1. To provide specialist assessment, therapy, management and evaluation to a group of clients with disorders of communication and swallowing secondary to an acquired condition and / or mental health diagnosis, working in the most appropriate setting on an individual or group basis.
2. To independently manage and prioritise a clinical caseload of adults with a diverse range of communication and swallowing difficulties.
3. To work jointly with colleagues and multidisciplinary team members, making referrals to other professionals when required.
4. To assess, treat and evaluate therapy to clients with acquired dysphagia and dysphagia secondary to a mental health diagnosis with support from senior colleagues as required.
5. To refer clients for videofluoroscopy with support from senior colleagues as required.
6. To provide advice and support to clients, carers and volunteers on communication and swallowing difficulties.
7. To develop and evaluate training for clients, carers, and a range of professionals on communication and swallowing difficulties, and the role of speech and language therapy.
8. To provide support and supervision to speech and language therapy students.
9. To contribute and participate in service development projects, audit, QI projects.
