An excellent opportunity has become available to be part of our Neighbourhood Mental Health Team in Stepney and Wapping. This is an ideal opportunity for a qualified mental health nurse to expand in their skills, expertise, multi-agency liaisons and autonomous practice.

The Tower Hamlets Stepney and Wapping Neighbourhood Mental Health Team is a well-established team within East London Foundation Trust and operates as an integrated multi-disciplinary team with staff from both the East London NHS Foundation Trust and the London Borough of Tower Hamlets working together to provide a high quality service for people with severe and enduring mental health needs this is inline with Transformation Project. The team actively works in partnership with service users, carers, the Local Authority and with other statutory services and local voluntary groups.

Main duties of the job

The post holder will be responsible :

To be an active member of the multi-disciplinary team providing assessment, care and

support to Adult presenting with mental health issues within the neighbourhood of Stepney and Wapping.

To conduct comprehensive and high quality holistic Health and Social Care (Care Act 2014) assessments including safeguarding when required.

To act as a care co-ordinator for a caseload of service users and develop, implement and review

individualised recovery focused care and support plans and commissioned packages of care.

To conduct carers assessments and offer support and advice to carers and relatives.

To promote the needs of service users and offer and provide training and education accordingly.

To operate as a Duty Worker on rotation across the team in line with Mental Health Transformation Project, responding to emergencies, processing referrals and presenting at referrals meetings of your assessment.

To assist the Senior Practitioners and the Operational Lead in the management of the service and deputise in their absence, where appropriate.
