Job summary
We are seeking an experience nurse to support the Specialist Palliative Care Nursing Team to provide direct palliative care to a caseload of patients. You will be responding to unplanned events and working alongside the Specialist nurses and wider community colleagues to provide palliative care support to patients within Mid Bedfordshire.
Experience in the field of adult palliative and community services is desirable
We are looking for someone who is dynamic and motivated and has the confidence to support and develop individuals to deliver consistently high quality services. Excellent communication and time management skills, and sound clinical knowledge, is essential. A flexible approach is required to accommodate the changing needs of the service along with the drive to influence service development.
Main duties of the job
This is an excellent opportunity to develop your knowledge and skills in Palliative care. You will work closely with the team to support and facilitate end of life care at home. You will also have the opportunity to be involved in facilitating self-care for patients with long term conditions, health promotion and innovative activities and projects that enhance patient service delivery and staff wellbeing. You will be responsible for a specified group of patients, with the support of the Specialist Palliative care Nurses and respond to calls within Urgent Community Response to ensure people are supported to remain at home and to prevent unnecessary hospital admissions.
This a fulltime 37.5 hour per week post, part time hours would be considered
You will be aligned to the Specialist Palliative Care team who work from bases in Mid Beds: Ampthill, Biggleswade and Shefford
