You will be part of The Mid Beds Community Mental Health Services. Our team is part of the Mental Health Transformation of services and our new blended team where our voluntary service colleagues are fully embedded within the team. Main duties of the job
You will work as an autonomous practitioner within a dedicated Multidisciplinary Hub. If you are committed, relish new challenges and strives for excellence, this post is for you.
You will hold a caseload and work within the principles of the agreed model of care for the service as laid down in the Operational Policy.
This will involve the assessment, care planning, implementation and evaluation of the care provided and will liaise with all agencies involved with the clients. The role includes being part of a duty rota, screening referrals and participating in Quality Improvement (QI) projects within the Team.
You will also take part in the Transformation Agenda in line with The NHS Long Term Plan, which describes:
New community-based offer which will include access to psychological therapies, improved physical health care, employment support, personalised and trauma-informed care, medicines management and support for self-harm and co-existing substance use and proactive work to address racial disparities.
