Nurse Prescriber
Greenhouse Practice is a general practice managed by the East London Foundation Trust (ELFT). It registers patients who are homeless (this encompasses street homeless, hostel dwellers, those in temporary accommodation and those who have no secure housing). Patients register with the practice and are offered a full range of primary care services. The practice is responsible for the delivery of primary care both in the practice and in an outreach setting. In an outreach setting, the practice provides clinics in temporary accommodation settings such as hotels, hostels and shelters looking after a combination of rough sleepers and vulnerably housed service users. We also provide clinics and sessions on a bus in the City & Hackney area where our clinical team will be required to work on a rota basis.
Nurse Prescriber requires a combination of working in the practice and providing outreach sessions for the practice in an integrated care setting. This will ensure that we can deliver an integrated care model with continuity of care to a population facing many inequalities. The successful candidate will be part of the Greenhouse team working to tackle these inequalities and will be working closely alongside The Greenhouse Team which includes GPs, lead Nurse and HCAs.
Main duties of the job
- Nurse Prescriber is responsible for ensuring the delivery of safe and effective nursing care to the practice population.
- Nurse Prescriber is accountable for nursing service delivery and they will lead and manage the Health Care Assistant.
- Clinically, the focus of the role is the delivery of evidence-based care for patients with long-term conditions, wound care and management and preventative nursing interventions to all patients.
- As an autonomous practitioner the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making
- Nurse Prescriber will demonstrate safe, clinical decision-making and expert care for patients within the general practice.
- Nurse Prescriber will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures and providing leadership and direction for the nursing team.
- Nurse Prescriber will build effective working relationships with other health care professionals.
The post holder will work within the multi-disciplinary team to deliver excellent primary health care services within the practice.
