Job summary

Bevan ward is a modern 15 bedded male PICU at the City and Hackney Centre for Mental Health. This Unit provides expert, supportive, short term, individualised, intensive care for service users aged 18 to 65, resident in the Boroughs of City & Hackney, who are in an acutely disturbed phase of a serious mental disorder, resulting in increased risks that require them to be treated in a controlled environment for a brief period of time.

The ward aims to run at or below 85% bed occupancy (circa 12 patients) reflecting a balance between transfers in and out and occasionally direct discharge to the community. This is not the usual pathway but can be appropriate, as required on a case by case basis. For example if a patient historically can't cope with the reduced boundaries on an open ward so discharge direct from PICU can be more useful - this is the exception rather than the norm though. There is typically a turnover of 2-3 patients per week.

The unit is an important and integral part of the service user pathway, which reflects the Trust's vision of care that requires a fundamental respect for individual needs. Care is delivered on the basis of the principle of therapeutic intensity, rather than containment alone. The Unit provides a therapeutic milieu with a range of interventions with an emphasis on intensive multidisciplinary treatment, safety, individualised care, professional rapport and staff development.

Main duties of the job

The post holder will be responsible for the care and management of all patients who are admitted to Bevan Ward until their transfer to locality ward, or (more rarely) discharge. Effective communication with GPs, CMHTs, fellow consultants, the pharmacy team, the Mental Health Act Administrators, the inpatient wards and the HTT is essential.

The post holder is expected to manage within the available beds to avoid the use of extra-contractual referrals and independent-sector beds.

The post holder will be expected to work to achieve key performance indicators such as, risk assessment and completion of care planning documentation.

The most usual care pathways to PICU are:

1. From the community:

  • Sec 37/41 service users recalled to Bevan Ward (on average there are between zero and two restricted patients on the ward at any one time)
  • Service users whose clinical presentation is too chaotic/violent/risky, who could not be managed on an open ward
  • Service users who are admitted straight to seclusion

2. From Prison:

  • All service users on remand/convicted, transferred by MoJ warrant, detained under any section of Part III of the MHA.

3. From the acute wards:

  • Service users whose clinical presentation is too chaotic/violent/risky, who could not be managed on an open ward
  • Service users who are at risk of persistently absconding from an acute ward
