This role is a fixed term maternity cover for 1 year, working within our Diana complex healthcare and palliative care team.
Working with a team of experienced nurses, play therapist and Psychologists, this role enhances the MDT care around some of our most complex children, aiming to keep them at home, living their best life.

Main duties of the job
To manage a clinical caseload ensuring continued responsibility for the assessment,planning, development, implementation and evaluation of programmes of care.
To work in partnership with other community/ acute based teams, clients and services in the public, private, educational and voluntary sectors.
To be able to provide line management for junior members of the team associate to caseload, including clinical supervision and appraisals.
To demonstrate areas of specialty thereby acting as a resource for other professionals and enhancing evidence-based practice for delivery of community based nursing care for children and young people with a variety of conditions, including life limiting and complex care.
To work with other members of CCNS to provide an out of hours on call rota-covering support for a terminally ill child.
To work flexibly within patients homes on a rotation of evenings, bank holidays and weekends as needed.
To work in Diana palliative care, and flexibly across community children's nursing service (CCNS), including Community children's nursing team (CCNT), Special School Nursing and Continuing care if the need arises.
