Job summary

We are seeking a motivated and dynamic Band 4 Assistant Practitioner to work within our Dunstable Primary Home Care Team. You will be based in Dunstable, however there is an expectation that staff will cover across the bases if needed.

Main duties of the job

This is an excellent opportunity to develop your knowledge and skills within the community nursing setting, with the possibility of being supported to complete your Nursing qualification. You will have the opportunity to be involved in facilitating self-care for patients with long-term conditions, health promotion and innovative activities and projects that enhance patient service delivery and staff wellbeing. You will also have the opportunity to be involved in Urgent Response and Virtual Wards, which is a focus of our teams

You must have a driving license and have access to a vehicle in order to carry out your duties. You will be required to work a shift pattern covering 7 days a week from 08:00-21:00.
