Senior Advanced Specialist Pharmacist for Triage Wards Newham

Job summary

  • The post holder will be expected to provide daily clinical pharmacy service to Ruby Triage and oversight over the second Triage ward Ivory supporting MDT with appropriate admissions and effective discharges. They will be responsible for implementing the Trust's clinical pharmacy standards as well as ensuring the clinical service is of high quality and patient focused.
  • They will ensure pharmacy presence in ward/clinical meetings and play an active role in implementing safe systems around the management of medication.
  • They will identify gaps and problem areas in the service and to support use of quality improvement methodology to make positive changes.
  • Post holder will work closely with the discharge co-ordinators to ensure a robust and effective discharge process from Ruby and Ivory Triage ward.
  • In addition the post holder will to be expected to work with the lead pharmacist , Clinical lead and the ELFT Medicine Safety Officers to develop and lead on Medicines Safety in Newham Directorate.
  • Post holder will have oversight of medicines related incident reports, working with stakeholders to co-ordinate and implement action plans.
  • Liaise with Newham MDT on matters involving medicine safety
  • To support with education & training of healthcare professionals on matters related to medicines.

Main duties of the job

  • 1. To support and coordinate a comprehensive clinical pharmacy and medicines management service to Ruby and Ivory Triage wards.
  • 2. To ensure the provision of a high quality, responsive and patient centered clinical pharmacy service to all Triage patients implementing the Trust's clinical pharmacy standards.
  • 3. To attend Ruby Triage ward management meetings including clinical risk and inpatient management meeting
  • 4. To support with audit and quality improvement projects for the Triage wards and Newham pharmacy alongside the lead and clinical lead pharmacist
  • 7. To support seamless care at discharge and be the link pharmacist for service users as they transfer between secondary and community mental health / primary care / PCN services
  • 8 To support with providing training on matters relating to medicines to MDT in Newham directorate
  • To work with stakeholders in embedding lessons learnt from medicine incidents supporting good medicine safety culture.
  • Post holder will have oversight of medicines related incident reports, lead on directorate medicine safety meeting whilst working with stakeholders to co-ordinate and implement action plans.
  • To support healthcare professional leads in Newham with education & training of healthcare professionals on medicines.

Essential

  • Vocational Masters Degree in Pharmacy (4 years or equivalent)
  • One year pre-registration training
  • Post registration training relevant to hospital pharmacy (substantial experience in a Mental Health setting)
  • Member of the General Pharmaceutical Council and/or Royal Pharmaceutical Society
  • Certificate in Clinical Pharmacy (or equivalent)
  • Diploma/MSc in Clinical Pharmacy (or equivalent or willing to work towards this)
  • Psychiatry qualification (or willing to work towards this)

  • Supplementary or Independent Prescriber (or willing to work towards this)
