Job summary
Band 6 Dual Diagnosis Community Mental Health Nurse:
The aim is to provide a proactive care approach, deliver a coordinated care and MDT to the hostels within the London Borough of Tower Hamlets. The role will include a range of activities addressing the client's health and well-being, the development of pathways into specialist services and improve the overall health inequalities via:
Coordinated care and personalised care and support plan
Identifying residents who are at risk of crisis
Engagement with primary care services, local authority, hostels, housing and other key partners
Main duties of the job
To work with the hostel social worker, hostel service managers and staff to deliver the care coordination approach
To work as a member of the multidisciplinary team in the management and treatment provision of a range of specialist services to hostel residents
Identify any gaps in dual diagnosis provision and provide examples of good practice to inform future pathway and service design and provide a case management role for identified dual diagnosis service users as appropriate.
When needed, to provide interventions to support resident's health and well-being such as harm minimisation approaches, motivational interviewing, relapse prevention, care planning, individual intervention, general health assessment, wound assessment and management, injecting site assessment, lifestyle assessment and physical health assessment including blood glucose, urine tests and others. The role includes using skills in mental health assessment and referral to appropriate mental health services and group work.
Work closely with primary care, housing, safeguarding teams (ASC), substance misuse, mission practice taking on/follow up referrals.
