Job summary

The Extended Primary Care Services enable the provision of multi-disciplinary care and support to vulnerable adults, in a range of community settings. Clients are referred primarily due to a physical disability and have complex medical and social needs. Therapy and support staff work in multi-disciplinary teams across Tower Hamlets, liaising closely with local GP's to provide the majority of community health services for adults living at home.

The aim of Extended Primary Care is to provide rehabilitation to clients in their own homes.

The rehabilitation support worker assists physiotherapists/occupational therapists in providing treatment to patients. You will be responsible for carrying out rehabilitation programs with patients, as devised by the physiotherapist/occupational therapist, to maximise patient's mobility and independence in daily activities in their own home and outdoors.

Main duties of the job

An opportunity has arisen for a motivated and enthusiastic Band 3 Rehab Support worker to join the Extended Primary Care Team within Community Health services Tower Hamlets , part of East London NHS Foundation Trust. This post offers an exciting opportunity for a Rehabilitation Support Worker with relevant experience to work within a community health setting and further develop their skills.

You will be closely working with Occupational Therapists and Physiotherapists as well as with the MDT. You will be assisting the physiotherapists/occupational therapists in providing treatment to patients. You will be responsible for carrying out rehabilitation programs with patients, as devised by the physiotherapist/occupational therapist, to maximise patients mobility and independence in daily activities in their own home.

You should have experience, working with physical health clients, basic knowledge of medical conditions. Effective time management and caseload management skills are essential, as are excellent communication skills and a commitment to the principles and practices of effective MDT working.
