Job summary

We are seeking a motivated and dynamic Band 5 Community Nurse to work in our Palliative Care team, within our North Locality. The successful candidate will work within the District Nurse Team and the focus of this role will be to provide all care to our End Of Life Care patients in their own homes, with the support of a Band 6 Senior Palliative Nurse

Main duties of the job

This is an excellent opportunity to develop your knowledge and skills in Palliative Care. You will work closely with the Band 6 nurse to support and facilitate end of life care at home. You will also have the opportunity to be involved in facilitating self-care for patients with long-term conditions, health promotion and innovative activities and projects that enhance patient service delivery and staff wellbeing. .You will also have the opportunity to be involved in Urgent Response and Virtual Wards, which is a focus of our teams.

Experience in the field of adult palliative and community services is essential.

You must have a driving license and have access to a vehicle in order to carry out your duties. You will be required to work a shift pattern covering 7 days a week from 08:00-21:00.
