Band 6 Occupational Therapist - Discharge Navigation
Part time/ Full time Hours. Part time working from home optional.
We are looking for an exceptional Occupational Therapist to join the Transfer of Care Hub team at the Royal London Hospital specialising in discharge. The Transfer of Care Hub team is a joint health and social care service aimed at facilitating discharges from the acute setting and from step down beds, based on the home first principle. The team prevents avoidable delays in patient discharges through coordination and planning in accordance with current government discharge guidelines. You will problem-solve to overcome challenges and work with the ward multidisciplinary teams, step down bed facilities in the community, senior management and external agencies as well as advocating on behalf of your patients.
The role is mainly desk-based and part-time working from home is optional. When on-site you will be based both on the wards and non-clinical areas, as well as visiting step-down bed facilities and providing very occasional home visits as required by the service. You will provide discharge advice and information on local authorities, voluntary agencies, and other help and support available to patients once discharged into the community.

About us
You will act as a discharge planning expert to the wards and step-down bedded units to ensure that people who do not require acute hospital care or step-down beds are discharged to their usual place of residence, rehabilitation units or residential/nursing home placements.
We are a 7 day service operating from 08:00 to 20:00 including bank holidays. Working pattern flexibility and can be discussed.
