Bedfordshire Accommodation and Rehabilitation Team BART

A Secondment Opportunity has arisen for a Band 6 Social Worker, Registered Mental Health Nurse or Occupational Therapist in the Bedfordshire Accommodation and Rehabilitation Team (BART).

BART are committed to working in partnership with ELFT mental health services, supported accommodation services, employment services, the voluntary sector to achieve robust, rounded rehabilitation services working within Bedfordshire and Luton. This is an exciting opportunity for a Social worker, Registered Mental Health Nurse or Occupational Therapist to join our small, experienced team for a 12 month secondment

The key priorities for the team are;

  1. To undertake a review of all Luton patients with SMI living in supported accommodation to help us to understand what sort of rehabilitation package of care might be needed to be to support move on and over what time period with a recovery focused pathway
  2. To support BLMK in the development of a sustainable accommodation strategy
  3. To gate keep all future applications to supported accommodation
  4. To offer enhanced support to facilitate community care for complex patients currently admitted to local mental health inpatient wards, which otherwise may need to be referred to specialist units
  5. To review and provide enhanced support to enable step down for patients currently in specialist placements.

Main duties of the job

This particular role will be based at Grove View Integrated Health and Care Hub in Dunstable. You will be working with members of the multidisciplinary team including CPN's, Doctors, Psychologists, Support Workers and Occupational Therapists.

You will endorse and act in accordance with the principles of personalisation within The Care Act 2014 to ensure that care and support is person centered and as far as possible places people in control of their own lives. In doing so they will carry out assessments of need in accordance with The Care Act, plan and deliver services and review outcomes with the individual, their personal networks and support providers.

You will be expected to work in partnership with service users and carers, identifying and utilizing strengths and working closely with other agencies and voluntary groups to ensure good outcomes which, as far as possible, maintain independence. You will need a good working knowledge of The Care Act, Mental Health Act and Mental Capacity Act/DOLS and other social care legislation.

Leading on reviews of individual patients are identified as a priority within Luton and the aims of these reviews will include;

  • Ensuring that the placement continues to meet the persons needs.
  • Identifying an appropriately supported, (ideally more independent) placement for the service user to move-on to in the future ideally in their area of origin (where desired and clinically indicated).
  • Identifying with the service user and the staff of the out of area placement clear goals for progression through the pathway being identified (e.g. managing medication more independently, self catering, budgeting).
  • Facilitating assessment by the potential move-on accommodation provider at an appropriate time.
  • Liaising with all parties, including family members/carers, and support the service user and family practically and emotionally through the assessment and move-on process, including visits, transitional leave and final move.
  • Continuing to review the new placement if out of area, or hand over case to local community mental health.
