Job summary
Bedford Community Mental Health teams (part of East London Foundation Trust) are seeking enthusiastic and motivated Mental Health Nurse to join our forward-thinking and dynamic team.
You as a post holder will hold a caseload and be working collaboratively with all the secondary mental health services including Primary Care Mental Health Link Practitioners, the primary care services, and the Voluntary and Private Sector to contribute to providing a Recovery focused model of care.
Main duties of the job
Key Responsibilities:
? To be responsible for an identified community caseload, ensuring high standard of professional conduct to comply with best practice and professional accountabilities.
- To promote a service user focussed service
- Participate in audits and survey relevant to the team
- To participate in development of new policies and procedures as required
- Ensure that clients receive a culturally competent service
- To co-ordinate the day-to-day activity and manage the workload of the Team
- To ensure the legal requirements of the Mental Health Act (1983) and subsequent amendments are adhered to
- To establish and maintain good relationships with service users, carers and voluntary sector colleagues
- To provide best evidence practice to service users which complies with legislation, policies and procedures and relevant quality, governance and local and national guidelines.
- Liaise with General Practice and Primary Care colleagues to promote partnership working
? Ensuring that the local demographics are taken into consideration for service provision and improvement

Essential
- Be responsible for the assessment, development, implementation and evaluation of programs of care for patients, incorporating risk assessment, to ensure safe and acceptable standards of care.
- To develop and maintain a high standard of practice based on assessment through the use of pscyho education, the Recovery Model and supportive clinical management.
- To have knowledge of a range of therapeutic interventions available to reduce distress for individuals experiencing an acute mental health crisis.
- Ability to work autonomously and within MDT setting.
- Be responsible for the maintenance and accurate recording of patient information in the documentation systems and to ensure the effective dissemination of patient information throughout the multidisciplinary team and partnership organisations.
- To attend clinical meeting in providing the team with relevant information in relation to the management of the HTT caseload.
Education

- Relevant Degree/Diploma, this may include registered nurse/therapist/psychologist etc.
- Registration with relevant UK Professional Governing body (NMC/HCPC etc.)
