LD and Autism Complex Care Lead
Band 7 - 37.5 hours per week
An exciting opportunity has arisen for an experienced practitioner to join a pilot initiative within ELFT, focusing on people with learning disabilities and / or autism, whose complex needs are supported within the TCP framework in Milton Keynes, on behalf of BLMK.
This is a role for a dynamic, registered practitioner who is committed to managing, system wide processes that prevent unnecessary admissions to mental health or secure settings, and support timely discharge and/or step-down to community settings.
You will be supported to develop your expertise and leadership skills within the role, to ensure that local mental health services undertake their responsibilities, to ensure that patients receive the right care and treatment when it is required.
Knowledge of local and national initiatives relating to Transforming Care Partnerships, is a key requirement of this job. Additionally, knowledge and understanding of the Mental Capacity Act, the Care Act and the Autism Act are also required.
You should have experience working in the field of health and social care and be able to demonstrate experience of positive partnership working. You will work alongside partners such as: Local mental health inpatient areas, CMHTs and ALDTs, NHS EoE Provider Collaborative, People who have LD and / or, Autism, Carers, Families and Children's Services.

Main duties of the job
- The post holder will be the lead contact for people with learning disabilities and / or autism whose complex needs are supported within the Transforming Care framework in Bedfordshire.
- The post holder will be responsible for implementing and managing system wide processes that prevent unnecessary admissions to inpatient mental health or secure settings, and timely discharge and/or step-down to community settings.
- The post holder will be an experienced clinician who provides leadership and expertise to ensure that ELFT and its partners undertake their responsibilities, to ensure that patients receive the right care, in the right place and at the right time.
