Job summary

We are looking for an enthusiastic, skilled and competent mental health nurse, with an interest in the care of older adults, to work within our Diagnostic Memory Clinic.

The Tower Hamlets Memory Clinic provides a dynamic, innovative and responsive service for the population where dementia diagnosis rates are increasing. The clinic provides high quality comprehensive assessments and diagnosis for people experiencing cognitive decline by working actively in partnership with service users, carers and relatives. This includes quality post-diagnostic support and intervention that promotes independence, social inclusion and maintains quality of life.

The service works in partnership with other teams, statutory and voluntary agencies as well as universities and other research organisations.

Main duties of the job

You will be responsible for supporting a caseload of service users through the diagnostic pathway by providing comprehensive cognitive assessments and post-diagnostic support.

Essential

  • Registered Mental Health Nurse.
  • Educated to diploma or degree level
  • Evidence of continual professional development

Desirable

  • Higher Education qualification in Mental Health.
  • Trained Adult Safeguarding Enquiry officer or willing to undertake Enquiry Officer training.
  • ENB 998/ Mentorship Course or equivalent
  • Evidence of continual professional development related to cognitive impairment / dementia/frailty care.

Experience

Essential

  • Experience of working as a qualified nurse within a Memory Clinic or similar Dementia Diagnostic service.
  • Experience of community caseload management.
  • Experience in working with a diverse community in a complex environment.
  • Experience of working as part of a multi-professional or multi agency team
  • Experience of conducting holistic psycho-social assessments and providing comprehensive care and support plans based on individual need.

Desirable

  • Experience of working in an ethnically - diverse inner city area.
  • Experience of working with the older adult community.
  • Experience of working within an integrated Health and Social Care Team.
  • Experience of conducting standardised cognitive tests.
  • Experience of facilitating support groups.

Knowledge

  • Understanding of the Mental Health Act 1983(as amended in 2007).
  • Understanding of the Mental Capacity Act (2005) and how the act is used to support service users and carers / relatives.
  • Knowledge and understanding of the Care Act 2014.
  • Understanding of the principles of CPA and the Recovery Model.
  • Up to date knowledge of the current approaches to mental health nursing.
