Job summary

An exciting opportunity has arisen to join our innovative Advance Care Planning Team, supporting patients on the District Nursing caseload to receive person centred care in line with their wishes and preferences as they near the end of life.

The post holder will be working to support patients with advanced dementia and those who are important to them, to take part in discussions around their future care. They will be working closely with a range of other services to provide personalised and well co-ordinated care for patients as they near the end of life.

Main duties of the job

They will also have opportunities to work with patients who have a learning disability or learning difficulty, supporting them to express their preferences in relation to their future care, and ensuring that reasonable adjustments are in place to enable equitable access to care.

This is an opportunity to join a team who are passionate about supporting equitable access to end of life care and who were shortlisted last year for a HSJ award.

  • oRegistered Mental Health or Learning Disability Nurse with evidence of current NMC registration
  • oRelevant post-graduate short specialist courses at MSc level or working towards this
  • oEvidence of continuing professional development

Desirable

  • oPalliative or end of life care course
  • oLeadership training
  • oSupporting learning and assessment in practice course or equivalent

Experience

Essential

  • oRelevant experience of working with patients with advanced dementia and / or learning disabilities who may be nearing the end of life.
  • oExperience of integrated care and of liaising effectively with other organisations
  • oExperience of talking about advance care planning including death & dying
  • oRelevant leadership experience
