Job summary

Community Mental Health Nurse

North East Mental Health Team (PCN 2) and Stratford Mental Health Team (PCN 4) which are part of the Community Integrated Mental Health Service are each looking for a highly motivated and enthusiastic RMN who will work well within our MDT(Two CPNs requred). You will need to be skillful in assessing patients who may be presenting to mental health services for the first time with good biopsychosocial assessment skills.

Overall Purpose of the job:

To provide a consistently high standard of mental health care to people in Newham; to assess patients based on sound theory, principles and practice within a nursing context.

- To liaise with other care providers in Newham in order to facilitate a comprehensive service for individual service users.

- To promote and maintain a specific focus and interest in the care of people with mental health needs.

- To liaise and work closely with the teams attached ward in order to promote a seamless service between community and in-patient services.

Main duties of the job

The post holder will undertake regular liaison with all practices signed up to the PCNs Service. The post holder will identify a list of shared care patients and lead the meetings focussing on service specifications.

The post holder will work with the respective Consultant psychiatrist and Operation Team Lead to standardise the psychiatric intervention clinics provided to GP practices.

The post holder will provide regular written reports which will include analysis of complex data including referral patterns to CIMHS manager.

The post holder together with their clinical lead will support the GP practice in accessing a wider range of Mental Health Services.

The post holder will provide specialist advice, consultation and clinical supervision to allocated staff working in primary care.
