Working within a multi disciplinary Rough Sleeper team , in Path to Recovery - Bedfordshire Adult Addictions service
Main duties of the job
The Rough Sleepers Team is a multi-agency team providing delivering services to people with drug/alcohol problems and housing problems in Central Bedfordshire and Bedford Borough.Those presenting to the service are likely to have multiple social, physical health, and mental health problems.The post holder will provide specialist interventions as part of the Rough Sleepers team. They will manage a caseload of individual service users within a multidisciplinary and multiagency framework. This includes proactive engagement, assessment, care planning, risk mitigation, crisis planning and management and promoting advocacy and multiagency working.The post holder will provide leadership and case managementThe aims of the service are to improve access to, and maintenance of housing (or a tenancy) whilst improving overall wellbeing, and access to and engagement with appropriate statutory and 3rd sector support servicesKEY RESPONSIBILITIES1.
