Job summary

Do you have a passion for working with people who have a learning disability and supporting them to access health services when they need to?An opportunity has arisen for someone to join a well established team within the Health Facilitation Service, which is part of the Learning Disability Service in Bedfordshire and Luton, East London Foundation NHS Trust. (ELFT).

The role will be 15 hours a week over 2 days and is suitable for a dynamic and compassionate registered nurse who is committed to supporting the needs of adults with a learning disability who are accessing primary care services. You will need to be passionate about ensuring a high-quality, holistic approach to the health care and support provided to people with learning disabilities, in the community. You will be required to work autonomously and as part of a wider team and deliver on projects reducing health inequalities.

Main duties of the job

  • The post holder will be responsible for the ongoing functioning of the Health Facilitation Service within the Specialist Healthcare Team, ensuring local and national standards appropriate to the clinical specialty are attained.

  • The post holder will take a proactive role in: managing the day-to-day Health Facilitation Service in the Bedford Borough locality area, service development, management, development and education of staff, personal development, service user involvement, data quality and ensuring equality and diversity values underpin the working practice.

About us

The Health Facilitation Team sits as part of the Health Equalities Service and works closely with Acute Liaison Colleagues and the Sight andHearing Team. The Health Equalities Service works hard to reduce those barriers which people with a learning disability face on a daily basis andto provide health promotion to people and carers, as well as health and social care colleagues.The team is well established and looking for someone with passion and enthusiasm to work alongside colleagues within Bedford Borough two days a week.

You will be well supported during your time with the team and be able to benefit from regular supervision and team meetings as well as otheropportunities for self development.
