Band 6 - Senior Community Nurse North and South Locality - Tower Hamlets, London
Hours: Full-Time (37.5 hours)

We in the Extended Primary Care team, have an ambitious vision for the Tower Hamlets community health service. We are striving to deliver high standards of care at the point of entry into our services, with the aim of improving access to quality of life for all of our patients. Our staff are central to delivering the service vision, and we are keen to recruit high-caliber staff who will be offered the support required to fulfill their roles as well as the training and experience to further their skills.
You will be expected to have completed the District Nursing programme or have the willingness to complete the course. You will be a strong team player and an excellent communicator who is adaptable, committed, well organised and work well under pressure.
Having a sound clinical assessment skill and ability to effectively manage time is essential for the post.

Main duties of the job
Support & deliver care to patients who have been admitted to the Community HealthTeam Caseload and will co-ordinate with support the Team s approach to care deliveryon a day to day basis.
Assess plan and implement a plan of care for individual patients.
Improve clinical outcomes for patients by enabling them to function independently by increasing their choice to remain in their own home/community and reduce the need for, orprevent inappropriate hospital admission.
Support joint integrated working across Community Health Team and otherStakeholders.
Work closely with the Team Senior Nurse/AHP to ensure care delivery is of a highquality and meets required standards.
