Job summary

An exciting opportunity has arisen for a motivated and enthusiastic band 6 community mental health nurse to join the Community Mental Health Care for Older People in City and Hackney. Please indicate 3 reasons why you should be considered for this post.

Act as a care co-coordinator within the CPA framework and provide a client-centered care to patients under the Care Act 2014 framework. He/She will manage a defined caseload of cases independently, providing comprehensive assessment and intervention, evidencing problem solvingand clinical reasoning skills in line with evidence-based practice.

You must have at least 18 months post-registration experience, which should include working in community mental health care for older people. Effective time management and caseload management skills are essential, as are excellent communication skills and a commitment to the principles and practice of effective team working.

You would be joining a friendly and forward-thinking multi-disciplinary, integrated service which is committed to high standards of practice.

We will support you in developing your clinical and supervisory skills, and progress to developing management skills. We will also offer you the opportunity to participate in clinical governance activities and other policy and service developments. We are committed to developing staff through regular supervision, appraisals, support in CPD and opportunities for career progression including AMHP training.

Main duties of the job

Purpose of the Job:

To be an active member of the multi-disciplinary team providing assessment, care andsupport to older people presenting with mental health issues and / or cognitive impairment within the borough of Tower Hamlets.

To conduct comprehensive and high quality holistic Health and Social Care (Care Act 2014) assessments.

To act as a care co-ordinator for a caseload of service users and develop, implement and review individualised recovery focused care and support plans and commissioned packages of care.

To conduct carers assessments and offer support and advice to carers and relatives.

To promote the needs of service users and offer and provide training and education accordingly.

To act as a duty officer in line with the service duty rota, responding to emergencies and processing referrals.

To assist the Senior Practitioners and the Operational Lead in the management of the service and deputise in their absence, where appropriate.
