Job summary
We are looking for an experienced and enthusiastic Registered Mental Health nurse with leadership and supervisory skills, and specialist interest in the care of older adults to join our Newham Diagnostic Memory Clinic as Senior Nurse Practitioner/Clinical Team Leader.
As a well-established Team with motivated, proactive and supportive team members, we care for service users with complex needs, including severe and enduring mental health needs and memory problems, requiring us to provide skilled interventions through the integrated Health and Social Care model of care.
We are currently undergoing an exciting time of transformation, working in partnership with our service users, their carers, Primary Care, Local Authority, voluntary organisations and other stakeholders in a drive to continue our goal of providing inclusive and high quality care while developing our service beyond the traditional scope of health and social care.
Main duties of the job
To support us in this process, the successful candidate will be working alongside the Operational Team Lead to provide day-to-day management and clinical leadership in the Diagnostic Memory Clinic, supporting the team to conduct memory, social care and other health needs assessments of people referred to the Service for memory assessment and making complex decisions regarding the planning, development and monitoring of post diagnostic care and support.
The Team promotes opportunities for professional development through regular supervision, access to trainings, including managerial and leadership and quality improvement (QI) projects.
