Job summary

We are looking for a motivated and flexible Community Psychiatric Nurse to join our well established specialist team. Experience and enthusiasm for working with service users who have severe and enduing mental health needs is essential.

The City & Hackney Rehabilitation & Recovery Service is a specialist community mental health team which offers care co-ordination for service users with severe and enduring mental health needs living in Hackney. In addition, we carry out regular placement reviews and support service users to move through the Mental Supported Accommodation pathway. The teams focus is on building on service users' strengths and confidence and developing their skills. The team is also responsible for providing the City & Hackney Clozapine Clinic. The nurses in the community team provide support to the clozapine clinic when necessary to cover leave. This is an ideal opportunity for a qualified professional to expand in their skills, expertise, and autonomous practice. The post holder will carry a caseload that is split between care co-ordination and placement review. They will have the opportunity to develop specialist knowledge and expertise in clozapine, community rehabilitation, housing, and applying legalisation including the Care Act, MHA and 117 aftercare, capacity, and safeguarding in complex cases. Main duties of the job

To independently manage a caseload of allocated service users with complex mental health needs and act as a care co-ordinator and case Manager to service users

To provide assessments of the service users placement, how their accommodation is meeting their needs and whether there are any unmet needs; presenting this to placement review meetings as required.

Where appropriate, support service users to move accommodations taking into account practical considerations and support needs through liaising with all relevant organisations and professionals i.e. accommodation providers, care co-ordinating team, GP etc.

Undertake the necessary training to provide cover in the clozapine clinic, including taking bloods, and to provide clinical cover to the clozapine clinic as and when necessary.

To undertake supervisory and appraisal responsibilities for allocated team members.

To act as fieldwork educator to students on practice placement

Essential

  • RMN Educated to degree level
  • Recognised post graduate qualifications in care of the adult with mental health problems

  • Significant post-qualification experience, which must be within mental health services
