An exciting opportunity has arisen for two Registered Mental Health Nurses to join a progressive CMHT in Luton. The Wardown CMHT is a dynamic and accredited Team (Service Users Accreditation -Gold Award) focused on Patients and Staff satisfaction. The team is under new leadershipand is working in partnership with the transformation programme which aims for NHS, social care and third sector services to work together to make a greater positive impact on the health and lives of groups and communities of people living with mental ill-health. Through providing more integrated care across mental and physical health services, and across health, social care, the voluntary sector and wider services.
Main duties of the job
The applicant will be expected to assess, follow up, provide pathway interventions and provide timely discharge. You will need to be enthusiastic and highly motivated with experience in assessments and delivery of care for patients from either inpatient or community setting. You will have access to clinical supervision within the team and will work closely with other professionals in the multi-disciplinary team to provide care co-ordination responsibilities for a defined caseload.
You will have experience of working in a community based setting. You will be responsible for the delivery of care to an allocated caseload of clients living in the Community experiencing, or recovering from, mental health difficulties. You will function in you role in our weekly Health and Clozapine Clinic as required and with responsibility of organising and managing the Clinics on the day.
You will provide direct support and advice to Service Users (Adults) and Carers, within the recovery framework, embracing the individual right to choice and control. You will ensure that appropriate health and social care services are commissioned as required. You will also assess, identify and evaluate Clients needs through comprehensive assessments, reviews, care planning and CPA in conjunction with other professionals.
