Are you a Band 6 Community Nurse who would like to join our dynamic team? We are seeking an ambitious, motivated, compassionate and experienced Band 6 nurse to support the changing face of community health care. As part of the Urgent Community Response and virtual ward initiative, your role will be essential in providing efficient, integrated, urgent care to our patients with care and respect.
You will be working as part of the Rapid Assessment Team to triage and assess referrals into the service; You will be accountable for initial history taking, clinical assessment, triage and onward referral.
Through face to face patient visits you will undertake a full holistic assessment, plan care with the patient at the centre and undertake physical examination of the patient with undifferentiated conditions.
Main duties of the job
You will be working closely with other members of the MDT and partners to formulate a management plan that will support the patient to remain in the community.
We are looking for an enthusiastic and motivated nurse with community experience, who is passionate about delivering high quality care. Excellent communication, time management skills and sound clinical knowledge, is essential. A flexible approach is required to accommodate the changing needs of the service along with the drive to influence service development.

- Work in partnership with other colleagues across primary and secondary care and other related services in order to provide a holistic care package designed to meet the specific needs of the individual, to facilitate hospital discharge and or prevent unnecessary hospital
- Work with the team lead in providing clinical leadership within the Community Nursing Service, providing line management and team support when
- Work with other agencies to deliver strategic national and local health
You must have a driving license and have access to a vehicle in order to carry out your duties.
You will be required to work a shift pattern covering 7 days a week.
Service hours are 08:00 21:00 with the core service hours being 09:00 19:00
