Band 6 - Continuing Health Care Nurse
We have an ambitious vision for the Tower Hamlets Continuing Health Care Team. We are striving to deliver high standards of care with the aim of improving access to services and the quality of life for all of our patients.

The role will involve undertaking comprehensive health assessments for patients who are 18 years of age and over to determine eligibility for NHS Continuing Healthcare & NHS Funded Nursing Care.
Main duties of the job
You will review and case manage Patients with complex health who are eligible for NHS Continuing Healthcare, supporting them within their own homes and in the community/nursing homes.
- Experience of working in partnership with other agencies as part of a multi-disciplinary team.
- Awareness of different funding streams available for care.
- Ability to support and manage junior staff within the team
The post holder will be expected to have strong managerial skills, an excellent communicator who is adaptable, committed, well organised and work well under pressure.
