Job summary
Health E1 is a general practice managed by the East London Foundation Trust (ELFT). It registers patients who are homeless (this encompasses street homeless, hostel dwellers, those in temporary accommodation and those who have no secure housing). Patients register with the practice and are offered a full range of primary care services. We are a teaching practice and regularly have students on placement. We are part of PCN Network 2, which is a longstanding well-established network, and we work closely with our network colleagues.
The remit of the service is to provide a primary health care service for homeless people in Tower Hamlets. The service works alongside an out-reach service to hostels and centers for the homeless offering health checks and health promotion. The service also works alongside the Pathway Homeless Team at the Royal London Hospital which in-reaches homeless patients admitted to the hospital. It is successful in reducing client use of secondary care.
Main duties of the job
It is intended that the Clinical Leadership will work together as a senior team to assure expectations about clinical quality are managed and a culture of continuous improvement is supported. The leadership will ensure that unit costs are managed and resources effectively allocated. The leadership will be responsible for attending key meetings with commissioners and stakeholders. The leadership will be members of the APMs Operational Meeting held monthly to consider quality and performance and will attend the bi-monthly Quality and Performance meetings.
In addition, the Lead GP will be the line manager of the other GPs in the practice as well as the clinical lead GP in the Pathway Homeless Team at the Royal London Hospital ensuring their competence and the quality of their work in pursuit of the service aims. This responsibility will include monthly supervision meetings and the annual trust appraisal and ensuring that doctors undertake Statutory & Mandatory training.
The job plan incorporates four clinical sessions providing GP services in the Homeless practices or partner agencies.
